Only a very limited number of authorized personnel will have access to your registry information, and this information is not stored in the same electronic repository as the survey questions you answer. If you opt into sharing your information with our research partners, any identifying information that you authorize us to share will be handled carefully and in compliance with healthcare privacy laws.
